شكستگيهاي ديستال استخوان زند زبرين از آسيبهاي شايع ناحيه مچ دست هستند. اين شكستگيها اغلب در اثر سقوط ايجاد ميشوند و ميتوانند موجب كاهش عملكرد ساعد و مچ دست، آسيب به ليگامانها و درد شوند. تثبيت اين شكستگيها با پلاك وولار و با قرارگيري پيچهاي قفلشونده در نواحي ديستال و پروگزيمال، باعث افزايش پايداري و تسريع روند ترميم ميشود. پيچهاي ديستال در برخي موارد بهصورت بايكورتيكال از هر دو لايه متراكم استخوان عبور ميكنند. اين امر ميتواند با احتمال بيرونزدگي پيچ از سطح دورسال و آسيب به تاندونهاي اكستنسور همراه باشد. اين آسيبها معمولاً با درد، تورم، تأخير در ترميم و عفونت همراه بوده و نارضايتي بيماران را به دنبال دارند. ازاينرو، انتخاب پيچهاي كوتاهتر با شرط امكان برقراري پايداري لازم در ناحيهٔ شكستگي پيشنهاد ميشوند.
هدف اين پژوهش، ارزيابي تأثير دو طول پيچ برابر با %100 فاصله بين دو قشر و %75 اين فاصله در پلاك گذاري وولار براي شكستگي هاي خارج مفصلي ديستال زندزبرين، به روش اجزاي محدود است. در اين پژوهش دو مدل استخوان سالم و مبتلا به پوكي استخوان موردبررسي قرار گرفتند. بدين منظور، مدل سهبعدي استخوان زند زبرين بر اساس تصاوير سيتياسكن بازسازي و در نرمافزارهاي ميميكس و تريمتيك پردازش شد. در مرحلهي بعد، گپ شكستگي در نرم افزار تريمتيك ايجاد شد. طراحي پيچهاي قفلشونده در نرم افزار ساليدوركس صورت گرفت و بعد از آن در نرمافزار تريمتيك پيچها همراه با پلاك، در ناحيهي وولار بر روي استخوان قرار دادهشدند. سپس شرايط بارگذاري شامل نيروي فشاري محوري، فشار مفصلي با هدف شبيهسازي نيروي واردشده از سمت مفاصل مچ دست، نيروهاي خمشي دورسال و وولار در نرمافزار آباكوس شبيهسازي گرديد. سفتي و پايداري اوليهي مدل هاي تثبيتشده با پلاك قفلشونده وولار سنجيده شد. بيشينه مقادير تنش بهدستآمده نشان داد با كاهش طول پيچ، سهم انتقال نيرو به استخوان متراكم و بخشهاي تثبيتكننده افزايش مييابد. همچنين جابهجايي گپ شكستگي با استفاده از طول پيچ كوتاهتر از %5 تا %9 افزايش يافت كه ميتواند در نگاه نخست، موجب ترديد در استفاده از پيچهاي 75 درصدي شود. با اين حال، با بررسي نتايج آزمون تي زوجي ميتوان دريافت كه در نمونههاي مورد ارزيابي در هر دو مدل سالم و پوكي استخوان، تفاوت معناداري بين دو طول پيچ در بارگذاريها و پارامترهاي مورد ارزيابي وجود ندارد. اين نتايج نشان ميدهد پيچهاي با طول %75 ميتوانند بدون كاهش معنادار در سفتي و پايداري مكانيكي، بهعنوان جايگزيني مناسب بهجاي پيچها با طول %100 فاصلهي بين قشري استفاده شوند و خطر آسيب تاندوني را كاهش دهند.

The impact of Distal Screw Length in Volar Plating on the Stiffness and Initial Stability of Distal Radius Fractures

Distal radius fractures are common wrist injuries. These fractures are often caused by falls and can cause decreased function of the forearm and wrist, ligament damage, and pain. Fixing these fractures with a volar plate and locking screws in the distal and proximal areas increases stability and accelerates the healing process. Distal screws in some cases pass bicortically through both layers of dense bone. This can be associated with the possibility of the screw protruding from the dorsal surface and damaging the extensor tendons. These injuries are usually associated with pain, swelling, delayed healing, and infection, leading to patient dissatisfaction. Therefore, shorter screws are recommended if they can provide the necessary stability in the fracture area. The aim of this study was to evaluate the effect of two screw lengths equal to 100% of the intercortical distance and 75% of this distance in volar plating for distal extra-articular radius fractures, using the finite element method. In this study, two models of healthy and osteoporotic bone were investigated. For this purpose, a 3D model of the radius was reconstructed based on CT scan images and processed in MIMICS and 3-MATIC software. In the next step, the fracture gap was created in 3-MATIC software. The locking screws were designed in SolidWorks software, and then the screws were placed on the bone in the volar region along with the plate in 3-MATIC software. Then, loading conditions including axial compressive force, joint pressure, simulation of force applied from the wrist joints, dorsal and volar bending forces were simulated in ABAQUS software. The initial stiffness and stability of the fixed models were measured with a volar locking plate. The findings showed that as the screw length decreases, the contribution of force transmission to dense bone and stabilizing segments increases. Also, fracture gap displacement increased when using shorter screws, which at first glance could lead to hesitation in using 75% screws. However, by examining the results of the paired t-test, it can be seen that in the evaluated samples in both healthy and osteoporosis models, there is no significant difference between the two screw lengths in the loadings and parameters evaluated. These results indicate that screws with a length of 75% can be used as a suitable alternative to screws with a length of 100% of the intercortical distance without significant reduction in stiffness and mechanical stability, reducing the risk of tendon injury.
