Abstract The present study aimed to adapt games an‎d activities based on the Total Physical Response (TPR) method an‎d compare its effectiveness with the Picture Exchange Communication System (PECS) in improving communication skills, receptive language, an‎d expressive language among children with autism spectrum disorder (ASD). This study employed a mixed-methods design (qualitative–quantitative). In the qualitative phase, the components of the TPR method were identified through a targeted documentary review an‎d research synthesis of scientific literature published between 2013 an‎d 2024. Based on these components, a 22-session intervention protocol was developed. The content validity of the protocol was examined an‎d confirmed using the Delphi technique with a panel of 10 experts (CVI = 0.90). In the quantitative phase, a quasi-experimental pretest–posttest design with a control group was employed. The study population consisted of all children aged 6 to 12 years diagnosed with ASD in Qom, Iran, during the 2023–2024 academic year. Participants were selec‎ted through convenience sampling from eligible autism schools. A total of 33 children (30 boys an‎d 3 girls) who met the inclusion criteria were ran‎domly assigned to three groups of 11 participants each: the first experimental group (TPR), the second experimental group (PECS), an‎d a control group (waiting list). Research instruments included the Test of Language Development–Primary, Third Edition (TOLD-P:3; Newcomer & Hammill, 1997) an‎d the Gilliam Autism Rating Scale–Third Edition (GARS-3; Gilliam, 2013). Data were analyzed using analysis of covariance (ANCOVA) in SPSS version 27. The results of ANCOVA indicated significant differences among the groups (TPR, PECS, an‎d control) in communication skills, receptive language, an‎d expressive language (p <‎ .05). Both TPR an‎d PECS were associated with improvements in communication skills an‎d receptive an‎d expressive language compared with the control group. Furthermore, Bonferroni post hoc comparisons revealed no significant differences between the TPR an‎d PECS groups in communication skills, except for maladaptive speech (p ‎> .05). However, significant differences were found between the two interventions in receptive an‎d expressive language outcomes (p <‎ .05), with PECS demonstrating greater effectiveness than TPR. Overall, the findings suggest that both TPR an‎d PECS may contribute to improvements in communication an‎d language abilities among children with ASD; however, PECS demonstrated greater effectiveness in enhancing receptive an‎d expressive language skills. Therefore, parents an‎d practitioners may consider incorporating visual communication tools an‎d movement-based activities into educational an‎d rehabilitation programs, taking into account the individual characteristics an‎d needs of children with ASD.
