Cancer is the third health problem in the world and the second health problem in Iran. The highest cancer rate belongs to breast cancer. In Iran, breast cancer has the highest rate of cancer among women. The present study was conducted with the aim of investigating the effect of positive mindfulness Program on distress tolerance, blood cortisol levels and heart rate variability in women with breast cancer. In terms of the purpose, this research was an applied and study, and in terms of the research method, it is one of the semi-experimental designs with a single-subject structure, in which the A-B design with follow-up was used. The statistical population of this research is made up of women with breast cancer, of which 3 women were selected by purposeful sampling and according to the inclusion criteria. The research tools were: Simons and Gaherʹs emotional distress tolerance scale, blood cortisol test which was done between 8 and 10 am fasting and measurement of heart rate variability using a smart watch. The intended intervention was positive mindfulness Program, which was implemented by the project manager. Before the start of the intervention, the performance of the subjects was examined for 3 sessions, and during this period, no intervention was provided to improve the performance of the subjects in these areas. In this way, the subjects’ baseline was determined. In the next step, positive mindfulness program was carried out for 8 sessions of one hour individually for the subjects, and at the end of the sessions, their performance was measured using research tools. After the completion of the intervention sessions, the performance of the subjects was examined again in 2 separate times. It is necessary to mention that blood cortisol levels were measured once in the baseline phase, once after the end of the intervention, and once after a month of follow-up due to the difficulty of implementation. For data analysis, visual analysis of graphs and trend index, stability index, percentage of non-overlapping data and percentage of overlapping data were used. The findings from the data analysis indicate the positive effects of this intervention on the evaluated variables in these women; So that the percentage of non-overlapping data for the first subject for anxiety tolerance and heart rate variability were 50% and 12.5% , for the second subject 87.5% and 75% and for the third subject 87.5% and 100%. Also, the percentage of recovery for blood cortisol level for the first, second and third subject was 10.7%, 11.1% and 19.8%. As a result, it can be acknowledged that positive mindfulness program had the least effect on the first subject, the most effect on the third subject, and a moderate effect on the second subject.
